Disruption Incident Reporting System

The Disruption Incident Reporting System (DIRS) has been implemented to ensure the timely and accurate reporting of all known disruptions of the Government of Alberta's essential services.

This system is designed and managed by Consequence Management Programs, Alberta Emergency Management Agency, a branch of Alberta Municipal Affairs.

The DIRS is an important element in the mitigation of potential disruptions. Not all threats to our essential services can be eliminated; however, through the use of systems such as the DIRS, our ability to implement controls upon risks will increase substantially.

The DIRS will provide all stakeholders the means to record and track disruptions and most importantly, ensure lessons learned can be documented for follow-up.

Submitting Reports:

Government employees are encouraged to complete the DIRS and send completed forms to their Ministry Business Continuity Planning Coordinator or manager.

Use the following steps to complete the 'Disruption Incident Report' form:

  1. Open Disruption Incident Report Form as a template to fill in electronically.
  2. Fill out the form.
  3. Perform 'File-Save As' to your hard drive.
  4. Print the form and send it to AEMA at 14515 - 122 Avenue, Edmonton, AB, T5L 2W4 ior fax it attention "Business Continuity Section" at 780-422-1549.
